Whether you choose to buy your Health Insurance Policy online or offline, you must submit the requested documents for our records, and to complete the KYC process. While online applicants may submit scanned, self-attested copies of their documents, offline applicants must carry photocopies to submit with their application, while visiting an insurance office branch.
- Duly filled Health Insurance application form with passport sized photographs attested on the form.
- A copy of identity proof documents for KYC purposes from a list of accepted documents like PAN, Aadhaar, Passport, Voter’s ID, Driver’s License, Employment ID, or any other document bearing your photograph.
- A copy of age-proof documents from a list of accepted documents like PAN, Aadhaar, Passport, Voter’s ID, Driver’s License, etc. In case of minors/dependent children, documents like their birth certificate and school leaving certificate are accepted as age proof.
- A copy of your address-proof documents for KYC and mailing purposes, from a list of accepted documents like Aadhaar, Passport, Voter’s ID, Driver’s License, rental lease, etc., bearing your temporary and/or permanent address.
- Copies of other necessary documents such as medical reports (in case of pre-existing illnesses) as requested.
